Why Do I Have to Register Before I Can Schedule?

At CHC, our services are not available to schedule for in the same way that you would schedule for an appointment at a private dental practice.

The reason for this is that the affordable services we offer must be applied for. But don't worry! There is no income limit that you have to meet in order to qualify to become a patient with us.

Depending on what county that you live in, certain household and financial demographic information and documentation will be required for you to qualify to become a patient.

You will provide this information and documentation in the online application portion of your registration process, which begins after you Register.

Why Do I Have to Complete An Application With All of My Personal Information and Household Income Documentation Before I Can Schedule?

Great question! At CHC, we are a non profit who strives to offer services at the lowest possible price for our clients to access. However, the price that we charge doesn't cover the cost of offering the services.

That remaining cost of service, which is not being charged to our clients, has to be made up somewhere in order for our doors to remain open and for the work that we do to continue.

This cost difference is made up by private donations, foundation and government-awarded grants which we apply for and utilize to subsidize the cost of the services that we offer to our clients.

Certain grant funding that is provided to us requires the collection of financial and demographic information from anyone who utilizes it.

In short, because CHC utilizes grants to subsidize the cost of ALL the services that we offer, anyone who uses our services is required to provide demographic information and sometimes financial documentation in exchange for access to our services.

We don't ask for your personal information because we need it to give you an appointment; we ask you because it is required from us in order to fund the services that you will take part in as a patient of CHC.
