Attention Existing Patients:

Grassroots Dental will be closed to seeing patients after Saturday, 12/23/2017, in order to complete our transition to a permanent office.

Our last day in our temporary office will be Saturday, 12/23/2017. We have a few options for a permanent office, but it may take us some time to get settled to resume seeing our patients. December will be spent seeing our existing patients, emergency patients and any new patients who need simple procedures that can be completed by 12/23. We will not be able to offer crowns, partials, bridges or any procedure that requires impressions  and lab work to be done, until we are settled in a permanent location. We will do our best to contact all existing patients by email when we are settled into our new location.

Records will be sent within 7-14 business days of your online release form being submitted.

PROGRAM

OVERVIEW

CHC offers a financial assistance program to help cover the cost of dental services for those who qualify. Applications are qualified based on household size, household income, and availability of funds. Applicants must note that submitting an application does not guarantee approval for participation in the program. Each application will be processed by a Case Manager, approved or disapproved, and then notified when they may begin accessing services.

2017 funding has been depleted.

We do not foresee any further financial assistance opportunities in 2017.
