With the advancement in technology, more and more types of healthcare visits can be done via a smart phone, tablet, or a computer. These advancements are creating hope for individuals and families who have a difficult time accessing care with a physician or healthcare provider.
To help in contributing a solution to this lack of access, CHC is working hard to source a telemedicine partner who can provide both primary care and chronic disease management (diabetes, high blood pressure, etc.). Our goal is to have a virtual primary and chronic care home for patients available on their smart devices to help them manage their health and live life to the fullest.
When this program becomes available, we will begin accepting applications for individuals to participate.

CHC is committed to helping our veterans to stay healthy and maintain their oral health to keep smiling long after their tours of duty are over. With more and more of our soldiers returning home, pressure on the VA grows.
For many veterans, getting the health and dental care that they need can be difficult. CHC is partnering with veterans groups across the nation in order to help by providing access to discount dental care and lab testing needed during medical treatment.
You can be a partner in helping as well. For a small monthly amount, you can help our veterans get the dental care they need along with other medical services. These funds would be able to be directed to our greatest need at the time. Sometimes our grant funding or donation programs are so specific that other needs that are just as important, go unfunded.
